Check, Register & Find Care

Start Here: Noticing a Concern

If you are worried about your child’s vision, hearing, movement, behaviour or learning, Care Finder is a safe first step.

This page helps you:

  1. Reflect on your child’s development through a guided self-check.

  2. Register basic information so we can better connect families to services.

  3. Explore a directory of services in Lagos that support children with disabilities.

Step 1

Self-check - Epic Early Development Checker

The self-checker is a short set of questions designed for caregivers. It:

  • Uses simple, everyday language.
  • Covers key areas such as seeing, hearing, moving, playing, talking, understanding and behaviour.
  • Takes only a few minutes to complete.

Important: This tool does not make a diagnosis. It helps you decide whether to seek further assessment and where to go.

Step 2

Register or Update Your Details

After completing the self-check, you can:

  • Create a basic profile for yourself and your child.
  • Save your responses securely so you do not have to repeat everything.
  • Give permission, where applicable, for EPIC providers to contact you or review your information when you attend a clinic or service.

Registration is optional but helps the system serve you better.

Step 3

Find Services Near You

The Care Finder directory will show you:

  • Primary health centres (PHCs) offering child disability screening.
  • General hospitals and specialist clinics with paediatric, rehabilitation or mental health services.
  • Community-based services, playsites and support groups (as they become available).
  • Contact details, opening hours and basic accessibility information where known.

You can search by location, type of service, child’s age or disability area (vision, hearing, mobility, learning).

Understanding Your Results

After you use the self-checker, you will receive a simple message, such as:

  • “No urgent concern identified – continue to watch and support your child.”
  • “We recommend a routine check with a health worker or teacher within the next few weeks.”
  • “We recommend you seek assessment as soon as possible – here are some services near you.”

These are guidance messages, not medical advice. They support you to make informed decisions.

Safety & Privacy

  • The self-checker does not replace seeing a doctor, nurse, therapist or other professional.
  • If your child is ill, in pain, injured or in danger, please seek emergency care immediately.
  • We are committed to protecting your privacy. Only authorised providers have access to your information, and only with your consent and in line with applicable laws and policies.
